The Foundling’s Partners for Change program works with young people and their families and utilizes the evidence-based Functional Family Therapy – Therapeutic Case Management (FFT-TCM) model to help families get motivated, understand their risks, and improve family stability.

Young people who are involved with the child welfare system face various serious risks that can affect their futures and livelihood for years to come. Partners for Change operates in Brooklyn and Queens and serves young people ages 0-18 (up to 21 if in foster care), and their families. Participants are referred to the program by the Administration of Children’s Services, hospitals, schools, nonprofit organizations, and the general community. The program aims to reduce and eliminate factors that lead to teens being placed in foster care, helping families stay stronger together.
