Communities That Care Evidence-Based Practice Education

The Foundling’s Communities That Care system uses an evidence-based model to engage neighborhood partners and improve life outcomes for families living in East Harlem.

Housed within The Foundling’s Home of Integrated Behavioral Health, our Communities That Care system works with a diverse coalition of partners within East Harlem – including local businesses, schools, health centers, religious institutions, other non-profits, and more – to create long-lasting community change.

The Communities That Care model’s evidence-based methodology has been proven to improve life outcomes for neighborhood residents, including better access to quality medical and mental health services, higher rates of educational success, reductions in violence and substance use, and other community improvements.

Drawing upon our track record and expertise in implementing research-backed models for social change, The Foundling maintains a leadership role throughout the process: creating and maintaining the coalition, directing community assessment and analysis, developing a plan with the coalition partners, and providing ongoing support as they work to engage the community.

Coming Soon:

The New York Foundling is currently conducting outreach in East Harlem to identify coalition partners, and is working with area schools to survey and assess negative risk factors and positive supports for neighborhood children and teenagers.

Beginning in Spring 2020, The Foundling will begin hosting regular coalition meetings and workshops to distribute resources, provide guidance, and implement a plan to mobilize the East Harlem community to enact positive change.

Isis loves spending time with her foster parent, Stephanie--whom she…

Isis loves spending time with her foster parent, Stephanie--whom she calls mom--and plans to go to college and possibly become a lawyer. Upbeat, fun, and energetic, Isis loves going to school, but found herself struggling to keep up with her classmates. That's when Jake entered Isis' life. The two began working together regularly and Jake quickly built a strong rapport with Isis and her younger sisters. While Jake may have taught Isis about fractions and decimals, Isis reminds Jake of the importance of positivity and gratitude.
